Avanti Senior Living, a revolutionary company that develops, operates and manages boutique-style senior living communities, and Eldermark, a nationwide software solutions provider for senior housing management, are leading the way with innovative technology in taking electronic healthcare records management to the next level. The two companies are preparing to launch an integrated electronic healthcare record-keeping system which will allow electronic healthcare records to be shared via a healthcare information exchange (HIE). This system is focused exclusively on senior care providers and state vendors. Previously, the records were simply stored at senior living communities and could not be shared. Eldermark is working with the Avanti team to create a customized virtual program with a senior living focus, as well as lifestyle apps for residents to use on their Avanti tablets. The new program will launch in 2015.

From a marketing standpoint, the system will track leads and prospects through move-in and will efficiently integrate all of their data into the system set up for residents. Avanti will do an assessment, noting specialized care needs, recommendations, preferences, current healthcare conditions and more. This data will be transferred into their electronic health record, where Avanti team members can maintain updated records. From there, the data is streamlined into the Eldermark HIE with both direct messaging and query capabilities which will allow Avanti team members to share the healthcare records with hospitals, clinicians and doctors, making the admissions and discharge processes more efficient. Avanti’s care team will continuously update data and track all aspects of a resident’s care program. This will help team members provide an individualized experience that is truly customized for each resident.

Additionally, the system will feature a distinct family portal, available on mobile devices and computers, which will give family members access to reports showcasing the care their family member is receiving, tools to view and break down billing in real-time and communication channels so they can check in with their loved ones via the residents’ custom Avanti tablets. The reporting system will also allow employees to print records to share with the resident’s family, showing tasks, medications, notes, visitors and more.

Eldermark will provide streamlined support, data entry, health data access, electronic medication tracking and efficiency through its software solutions. The program will enhance marketing functions, custom assessments of residents to provide individualized care, clinical forms management, EMAR, risk management, billing, general ledger, point of care, nurse call and health information exchange. Eldermark is certified in the Health Information Exchange and complies with standards set by the Affordable Care Act. The company has provided solutions to more than 1,000 communities nationwide.
