Case Management Program Supports Patient-Centric Care Coordination for Individuals with Complex Health Care Needs
ActiveHealth Management (ActiveHealth®) announced today that its case management program received three-year Case Management Accreditation from the National Committee for Quality Assurance (NCQA). Earning NCQA’s Case Management Accreditation is an indication that a case management program is dedicated to care coordination, patient-centeredness and supporting improved quality of care: helping patients get the right care.
